US first, MRI second for appendicitis in pregnancy — avoid ionizing radiation and gadolinium without forgoing a timely diagnosis.
Pregnant patients with suspected appendicitis are imaged with ultrasound first; when non-diagnostic — common as gestation advances — non-contrast MRI is the next study. CT is reserved for when MRI is unavailable and the diagnosis remains urgent.
