Do not image chronic stable headache with a normal neurologic exam; reassess only if the pattern changes or red flags emerge.
Chronic headache with a stable pattern, normal neurologic examination, and no red flags has a very low yield for significant intracranial pathology on neuroimaging. Routine CT or MRI in this population adds cost and incidental findings without changing management.
